Home from Hospital discharge

Age UK

Age UK Home from Hospital support for patients 18 years and over being discharged home. This service can provide up to 6 weeks practical and emotional support, onward referrals and signposting to local community services following discharge home from hospital. In addition Age UK provide a Home Response service for people aged 60 and over to help prevent a hospital admission from the person's own home living in BANES. The services include shopping and light meal preparation, collecting prescriptions, supporting wellbeing & safety, support to GP & other appointments, light cleaning, information & signposting and transport home from hospital, if eligible. Home Response Service is a chargeable service.

Age UK also provide HELP packs for people leaving hospital to support their safe discharge, The pack provides3 days of food and includes a follow up welfare phone calls over the following month to signpost and refer to local community services as required.

Bath Mind

Bath Mind support to people aged 18+ who have presenting to the Emergency Department based at the Royal United Hospital in Bath with a mental health crisis . They offer trauma-informed person-centred support to people with mental health crisis behaviours, low mood, depression and anxiety. They also offer places of safety for people needing mental health crisis support to avoid hospital admittance or after discharge from the hospital and before returning home (24hrs support).

Carers Centre

The Carers’ Centre offer a range of services to help unpaid carers focus on their health and wellbeing, connect with others in a similar situation and navigate the challenges of caring for someone. They offer a support line, in-depth support sessions, specific support for carers of loved ones with dementia on leaving hospital, planning for emergencies, and wellbeing activities to take a break from caring and meet others. They support both young and adult carers, from the age of 5 upwards.

Developing Health and Independence (DHI)

DHI's home support know as Reach is a partnership between DHI, Stonham (part of HomeGroup) and Clean Slate. They provide a combination of practical and emotional support to help people live independently and develop useful life skills. The Homeless in Hospital service ensures that rough sleepers or those at risk of rough sleeping are not discharged back to the street.

Stroke Association

The Stroke Association supports people to help them identify their personal support needs and priorities. The service can help people rebuild confidence and independence, and rebuild their lives after stroke. There is a dedicated co-ordinator who will make contact with the person to identify how they are managing with daily activities, and any additional support that is required.

We Care Home Improvements

We Care Home Improvements provides help to homeowners and private tenants. Referrals need to be made by a health professional. Support includes minor repairs, such as repairing a leaking tap, through to help when you leave hospital or advice on adapting your home including deep cleans and clutters and any work that is necessary to support discharge.
